Ok, so to find the definition of this 'phrase' I'm going to break it up with my own definitions and then add scriptures and Quotes.
Heart: The thing that keeps you alive. Physically and Spiritually.
Its where its said that deep feeling accor and where changes to your personality happen. Its where emotions happen and peace happen.
War: Turmoil, fighting, contention, conflict, hate.
So a Heart at War is literally you feelings battling against themselves.

PERSONAL DEFINITION:
When our hearts are at war, it might seem like there is no way to go back and oits all just a confusion and illusion and nothing seems real anymore, but really we just haven't gone to the right source. The way to get back onto the path and have peace, is through Jesus Christ.
When we don't have a heart at war, we are free to lead with Charity.
